The Care Management program is designed for the long-distance caregiver, adult children or other legal caretakers of older persons. Following a comprehensive assessment, a plan of care is designed to meet the ever-changing needs of each individual. Services are provided by a professional staff and could include: in-home assessments and evaluation of needs, coordination and monitoring of services, referral and follow-up, crisis intervention, family education and mediation.
The program aims to provide caregivers assurance that their loved ones are living as comfortably and independently as possible.
Services can be short-term or ongoing.
Insurance, Medicare and Medicaid are accepted for some programs. Services may be offered on a subsidized basis to those without insurance.Please contact Family Service for additional information.
